We are a team set up by the Foundation For People With Learning Disablilities and Cornwall People First
What are we going to do?
We are here to help people with learning disabilities who live in Cornwall:
To find out how they can get a Person Centred Plan
To answer your questions about Person Centred Planning
To tell you how Person Centred Planning could help you get to do the things you want to do
We will also keep the Cornwall Partnership Board informed of how many people are having person centred plans and if they think it is making a difference
