There is a new Person Centred Clearing House Project starting in Cornwall.

What are we?
We are a new team set up by the Foundation For People With Learning Disabilities and Cornwall People First.

What are we going to do?
We are here to help people with learning disabilities, living in Cornwall:

To find out how to get a Person Centred Plan.

To answer your questions about Person Centred Planning.

Tell you how Person Centred Planning could help you get to do the things you want to do.

We will also keep the Cornwall Partnership Board informed of how many people are having Person Centred Plans and if they think it is making a difference.

At present we are only working on Mondays but if you would like to get in touch please leave a message and we shall get back to you.
